I sure miss something but I can't see how to set "Item Image" and neither I see rj002/002 in my book metadata page.
Thanks in advance for any additional hint!

(In my account page - https://archive.org/details/@e_s_i_g_e_r_e_ - half of the titles display the right cover page, the other half internal pages... yet I can't tell any difference in uploading files).

If you go into the information for the item (Edit, Change the information). Go down to the item and change the "Format" field form JPEG to Item Image (it is a dropdown list).
